Insurance Information


We participate in Medicare and most commerical insurance plans. However, we encouarge you to confirm that we are a network provider for your specific plan. We are happy to bill your insurance company for you but we need a copy of your insurance card at the time of service. If the correct insurance information is not given to us, it may be necessary to bill you for the services rendered.


If a copayment is required from your insurance, we are required by your insurance to collect the copay at the time of service. Remember insurance companies often apply different copayment amounts for specialists. Please come prepared to pay this amount. Many insurance policies have a yearly deductible that must be met by the patient before the insurance will pay their percentage. Any unmet commercial insurance deductibles or coinsurances must be paid at the time of service for our surgical appointments. Our referral and authorization coordinator will be contacting you prior to your surgery appointment to discuss this with you.


Some health insurance policies allow you to make an appointment directly with a specialist while others require a referral from your primary care physician. Often these referrals have a number on them that your insurance company requires be attached to your claim. Other insurance companies only require a note from your primary care physician. If you have questions regarding referral requirements, please contact your primary care physician and/or your insurance carrier. Individual policies vary greatly.

Tricare insurance requires prior authorization and/or a referral. Referrals must be received prior to the appointment. If a referral/authorization has not been obtained you will need to reschedule your appointment or sign a waiver accepting full financial responsibility. If a referral/authorization has been obtained but you would like the physician to do additional procedure(s) or an office visit that has not already been authorized, you will need to sign a waiver. We will do our best to obtain the authorization; however, we cannot guarantee Tricare will accept the request. Please confirm with the receptionist or nurse that your visit and/or procedure has been authorized.
